Request for copies of all or any records of the late Simon Tembuyise Mndawe who died in detention under the security legislation at the John Vorster Square on 09 March 1983, the alleged cause of death was reported to be suicide by hanging, however there have been concerns about how he died.  The record must include but not limited to:

i. Records of inquest hearings;
ii. Post mortem reports;
iii. Truth and Reconciliation Commission files relevant.
iv. Copies of any or all medical reports prepared by the district surgeon, Dr Frans Viljoen who was responsible for examining Simon Tembuyise Mndawe’s at the time of his arrest, that details the extent of his injuries during the physical examination.
